There might be a chance they would accept the heading of the document to be ARC Conference Summary IF it included everything the PWN is supposed to. I would call someone in the special education compliance section of Kentucky's department of education and explain the situation and see what they say. If it is not sufficient to them, then at least you have the backing of the state when you reach back out to request a PWN.
